Home at Last (HAL)

Home at Last (HAL) is a program for seniors (55+) to help them get safely settled in at home after a hospital stay or emergency room visit. Services can include grocery shopping, getting prescriptions, personal care and help with other tasks.

 

Fees: the SWLHIN covers all costs for services associated with the HAL program.

 

The HAL program is delivered in partnership with the SWCCAC and Tillsonburg District Memorial Hospital. As of March, 2011, the team is participating in the "Alternate Level of Care (ALC) Long Stay Initiative", as lead by the SWLHIN, SWCCAC and hospital partners. The study will involve an in depth review of each ALC Long Stay client across the South West.
